Exit #17 is way off from the correct center point of that interchange and needs to be fixed.
Exit #138's split ramps look far enough apart that they should be split into separate points, 138 & 138A. |

I got 0.48 mi apart in WPTedit, just under the threshold.

Changes made to my local copy, to be submitted as part of my first "baby steps" try to use GitHub for highway data updates.
I went with yakra's suggestion to move 138 halfway within the split interchange, rather than add an extra waypoint. |
