Province names vs. Country?

What are the rules for determining how the end of a route is named when it enters or leaves the country?

I've noticed many routes say, for example, NY/ON or BC/WA (using the province name), but the north end of I-5 is labeled WA/Can (using the country).

WA/Can is totally wrong under current standards (newer than route creation)

USA/CAN is what it should be, though 2-digit code labels (eg WA/BC) are also allowed.

The Interstates' files were created before the standards were streamlined. As Si indicated, the point should be WA/BC or USA/Can.
